Dear Administrator or Director of Nursing:

The New York State Health Facilities Association/New York State Center for Assisted Living (NYSHFA/NYSCAL) has partnered with New York State Department of Environmental Conservation (NYSDEC) to assist in getting the message out about the agencies’ Tenth National Prescription Pill Take-Back Day on Saturday, September 26th, 2015.  This no-cost opportunity is a safe, convenient and responsible means for disposing of prescription drugs (non-controlled and controlled).  Adult care/assisted living and long term care facilities licensed by the New York State Department of Health (NYSDOH) and Bureau of Narcotic Enforcement (BNE) as Class 3a Institutional Dispensers, Limited will be eligible to participate in this event by:

  • Completing the Request for Approval of Disposal/Destruction of Controlled Substances: Instructions for the (DOH-2340) and the Controlled Substance Inventory Form (DOH-166).  Use “Surrender to Law Enforcement” as the method of disposal/ destruction.  Said forms must be received by the BNE no later than Friday, August 21, 2015.
  • Prior to the September 26th date, DEA officers (not BNE) will contact those 3a Institutional Dispenser, Limited facilities that have obtained BNE approval for disposal/destruction via “Surrender to Law Enforcement” and  DEA officers will schedule an on-site pick-up date/time for non-hazardous prescription drugs (non-controlled and controlled).  Do not include: Hydrocodone, hazardous waste drugs or liquids. Other than those mentioned, over-the-counter, prescription and controlled substances are allowed.
  • Box up and tape the unused, non-hazardous prescription drugs. Make a copy of the inventory list for the DEA officers.
  • DEA officers will collect all non-hazardous prescription drugs to maintain chain of custody.  Please keep in mind it could be up to a week or a few days prior to September 26th that your inventory will be collected and they will contact you with a date.  Click here to see the DEA link.
